Are Preexisting Conditions Covered?
Pre-existing conditions, can only be covered after the passage of a phased in time period of up to thirty-six consecutive months. During the “phase in period” such medical treatment cannot be for or related to any treatment, symptom or condition that existed prior to acceptance into the UFHS ministry. An illustration of the phase in period for pre-existing conditions is as follows: Extent of Coverage 1st twelve months no amounts will be shared. After 24 months 50% of eligible amounts will be shared. After 36 months 100 % of eligible amounts will be shared.
Basis of Pricing
UFHS ministry has sought to keep the pricing for members sharing as simple and straight forward as possible. The factors that go into pricing include age of the principal member, the number of people in the household and the selection of the program that the household participates in (i.e. Good, Better or Best). The method for calculating a member’s share is simple. Find the program that best suits your household (i.e. Good, Better or Best). Once this is determined then within that table for your selected program find the age range of the oldest person in the household and multiply the factor for that individual by the number of individuals to be included in the plan. For example, if a family of four (4) chooses the Best plan and the oldest individual in the household is 48 the monthly premium would be $1800 (i.e. Monthly premium for individual 48 years old under the Best plan is $450; $450 x 4 = $1,800.)
How Do I Join?
In order to join UFHS simply fill out the application form and complete and sign the subscription agreement and email it to unitedfaithhealthshare@gmail.com. A prospective member can also mail the completed application and signed subscription agreement to: United Faith Health Share Ministry, 944 A Kings Bay Road, #239, St. Mary's Ga. 31558. No money will be due and sharing of medical expenses will not begin until a completed application and signed subscription agreement are received and accepted by UFHS.
